What Maomao was saying wasn't that Lishu was considered a threat or could be a threat when she grew older, but that because the emperor didn't "visit" her at all she wasn't fulfilling her purpose for being there, and she could be targeted by someone who wanted to preserve Ah-Duo's position by creating a vancancy for that reason as perhaps the "unneeded" one among the concubines or maybe as a matter of honor

There was a one part I was confused in this episode and I'm sharing this just incase someone else was confused. I read a comment that the translation of the emperor and Ah-Duo being "foster siblings" is apparently not completely accurate and more accurate translation would be "milk-sibling." And if you're not sure what milk-siblings are, they are children who share a bond of being fed by the same wet nurse. When watchin the episode I was thinking about the relationships between the characters and the concept of them being foster siblings felt a little odd. I don't know if milk-siblings is any better, but it definitely fits the vibe of the show better imo.
