One of the most boring things I found in games with repeatable actions is that whenever the MC performs such an action the very same scene is displayed on screen. Some may vary lights or clothes (less common), but it's mostly the same scene.

Even with the limitations that my spare time puts on GP's development, I decided to make at least (which means more will be added in time) three different scenes randomly loaded when you'll perform a repeatable action (i.e. inviting Rita at the cinema).

This way you will see something a bit different each time. If I had the possibility I would add four times more than that, but I need to be self-aware about my limits.

General Practitioner's 0.0.6 will feat more than 600 (six-hundred) different images, that alone is some time-consuming work after all :D

Maybe a stupidt question, but is it possible to randomly change the persons clothes, without having to change the scene? Or would you need to create a whole new scene, each time a person have a new sweter or a new pair of pants on?

I would need to create a whole new scene and render it. So that's not gonna happen unfortunately, it will require ten times the time needed for development. Still, changing clothes will be a feature available soon, at least with some limitations. You will get to choose some clothing for yourself and the nurse(s) in your staff. Different clothing = different perks and that means Nurse Lawley will become a more interesting character soon :)
