You were correct, the marks on the legs were Tally marks. Its marking how many times Shizuku and the protag of the game have been... Involved.

I don’t know why they omitted this detail on the anime. On the manga, those are cold towels they are using. That’s why Marin was acting that way when Gojo was wiping her back.
