When she called Ryuken’s mom “Aunt Izumi”, she said “oba-sama” which can mean aunt but in some cases, especially this one, it is a honorific term used by a child to address older unrelated women. Therefore they are not related.

I have to admit, Isshin was smart. I never noticed until now, whenever the other soul reapers would stop by Ichigo’s house or would be in the area, Ichigo’s dad was never around and he must have taken Masaki’s last name in marriage. He made sure he was not discovered by name, face, or spiritual pressure. That was clever of him!
