It's coming out of her ass (not all the way) and going to an obvious place.  The parasite's crest of little stubs are squeezed against itself in the third panel, implying it's a very tight fit in there.

The spit and hair in the last panel gives us some idea how she is moving her body (direction and speed).

The second panel deliberately leave out the shadow on the eyeball.  This makes a character look more crazy or alert.  I also like how in the second panel she is trying to look at what is happening but can only feel and imagine.  That curiosity and struggle to make sense of things is something I think we can all relate to, and including things we can relate to helps the audience connect with the character.
