In my ABDL RP's my character Wilmfe kind of has a similar mentality. With her breastfeeding is absolutely linked to a full diaper. They won't get breastfeeding without a full diaper and they won't get their full diaper changed without a breastfeeding. I really like doing it because sometimes it's a torture to break them and sometimes it's a reward for embracing who they truly are. I often change the rules of "Wilmfe's nursery" depending on who I do it with. But that rule was one that pretty much always is there. Though there was one time where they thought breast feeding was a very, very hard 'no' so I replaced it with lap bottle feeding but that's still the same idea. While both are linked they are optional. Meaning that if they want a diaper change they will have to do a full breast feeding for the ones that hate it. While for the ones that like it they will have to get their diaper changed if they want a breastfeeding. During the breastfeeding Wilmfe always mushes the diaper and comments on how well used it is and how they are such a good baby for filling it up. She also talks a lot about how they have earned the breastfeeding. I kind of love it in both contexts. If they agree with her it's a really tender scene and if they don't it's really fun mocking.
