This came in as a request and was a lot of fun to create. In fact, it was so much fun, I'm creating a few more over on the blog right now. Once I'm done, all of these will be put into the book I'll be releasing at the end of this month.
Ghoul Fever
Ghouls are well-known for their paralyzing touch, but some ghouls also carry a horrible disease in their bite. As an optional rule, any creature bitten by a ghoul or ghast must succeed on a DC 10 Constitution saving throw or contract ghoul fever.
It takes 1d6 hours for the disease to manifest in an infected creature. Symptoms include hardening of the muscles and heavy bouts of nausea. Once every 24 hours, the infected creature's Dexterity and Constitution scores are both reduced by 1d4. The creature dies if this reduces either its Dexterity or Constitution to 0. A creature that dies from ghoul fever rises as a ghoul 1d4 hours later.
