Well I suppose we pretty much know for certain that Maomao's father is not her actually biological father - if he was the doctor in the inner Palace around the time/before Maomao was born - he would have been a eunuch, and maomao even refers to him at the end of the episode as that - so its almost as if she knows he isn't a biological father, but instead her father in the sense that he raised her. My money due to his age is grandfather.

I've been thinking that the woman wearing white in the OP is actually her mom. The hair is slightly different and she hugs Maomao at the end of the sequence before disappearing.
