Some shows air their pilot as their first episode, some don't. The original Star Trek had two pilots (which is very rare); they aired the second pilot as the third episode, but didn't air the original pilot until DECADES after the show finished its run. Buffy the Vampire Slayer's pilot was only half an hour long, so it was never aired at all. (I think it's a DVD extra.) Same with Avatar: The Last Airbender, its pilot was shorter than a regular episode so it was never aired.
