I forget the exact phrasing, but they imply here that the previous "doctor" that was "treating" the locals with her foot skin and leaves was the one that reported Lisa to the Church, in revenge for taking her business. The old lady shown here in particular was the final one who saw Dracula before he began his revenge. She's the one who greeted him and told him what happened, saying that she refused to believe Lisa was a witch.
