Not sure where you are going with this beyond the general. I think a tangent on his development that the Aunt lets get out of control. Maybe some enhancements that are to be "temporary". Him getting a GF thats likes him to be a "girl". Maybe he could join a club (or foced or seduced into one) where hiding his identity is hard and comical. Not sure if they do this in Japan but a class trip to Toyko by the school with shopping in the famous shopping district. A member of a Girl POP band that signs a deal with the company thinking he is a real girl.

I don't know if you want to go darker. But if you do the use of Mayumi as a strict teacher of behaving as a girl during a school break might work.
