Alluka was born male but sometime early on started to identify as female. Killua’s the only one that respects this while the rest of the family don’t. Another reason why Killua is best boi ^_^

Also there’s more to Alluka and Killua’s relationship that will be told in the coming episodes, but you’ll need to pay close attention to it bc I think they only explain it once.

Technically talking about Alluka's origin and why the characters refer to her under different pronouns is spoilers. The main take away is that Alluka's relationship with Killua is different than the rest of the family. He is protective over Alluka and refers to her as his sister.

Alluka has gender disphoria, everyone in the family aside from Killua refers to her as a boy though.

Sister or brother? the eternal question xD, I don't remember if they explain this or not, nor do I think it counts as some kind of spoiler, but what I interpret is that Alluca is male, but Killua, being the only one who loves him among his family, is the only one who refers to Alluca how she/he feels, or something like that

Alluka is a trans girl. Killua is the only one who respects her enough to use her preferred pronouns. The rest of the family don't even see her as human, but as an object which is why they constantly misgender her.

Every Zoldyck sibling was born biological male. But Alluka identifies as female. One who's identity I'm unsure of though is Killuas other 'brother' kalluto who we saw join the phantom troupe. I'm not sure if the character identifies as female but they're definitely designed with very feminine traits, particularly with the clothes they wear.

Alluka is a biological male. But Nanika, is a feminine spirit living inside her. Souls intertwined, that’s why he acts feminine.

I've never heard of this till now. I like it though. I mean I assumed an entity named 'something' probably existed without gender. I sorta thought it took on the femininity that Alluka's spirit embodied since they shared a body. I also thought maybe something didn't much care about that sort of thing so long as it had a vessel to act through. Something would have been happy taking on whatever form and expression that it found itself in or sharing.
