To answer your question at the end, when Amy goes through she says that the exterior has drifted in time and now if you go into the box then you come out in the past. That's why the Doctor kept trying to send people through, to maintain the timeline. :)

I did hear Amy explain that, but it still doesn't seem like it explains why there aren't multiple versions of each person. Shouldn't everyone who went through end up in the past each time? It seems like we should have been left with three Amys, two Rorys, and two Doctors at the end.

The first Amy to come through the door, was the same Amy that went into the TARDIS after slapping Rory. Since she had already come through the door, she wouldn't come through again. Then Amy and Rory came in, so the Doctor sent both of them in. Again since they had already come in they can't come in again. Then the same with the Doctor. It makes sense in my head, but my brain hurts trying to explain it lol.
