Random GM Tip: Encounter Distance

How does an encounter begin? It's easy to fall into a simple formula: The encounter begins at line of sight ("you see an orc") and immediately launches into an initiative check ("the orc tries to kill you"). There are usually minor variations on the line of sight (the orc opens the door, you open the door, the orc comes around the corner, etc.)
