I've always thought of the difference between Greyhawk and the Realms as arising from the fact that Oerth was created by wargamers, who thought first of how the kingdoms would meet each other on the battlefield, while the Realms were imagined by a boy who continued to refine his fantasy as a man (and continues to this day) but, at its core, is a boy's fantasy of sexy elf girls, mighty spells, and great cloud castles in the sky. For the most part, I think both have most of the same elements--the difference lies in which elements came about in service of the others in each setting.
