The interesting thing about hero names is that it's actually very common for a hero to be branded by the media before they can give their own name, but another equally common trope is to show that your young heroic protagonist has jumped the gun and not really though the hero thing through by having them be out on their first night fighting crime and someone's like "and who are you supposed to be?" And that's when they realize they haven't picked out a name yet.

I'm not an expert on the topic, but Deku in Japanese IS normally an insult, used to imply someone is 'useless' or 'can't do' something. However, there are alternative ways to interpret it that that instead mean 'useful' or 'can do'. Also as pointed out by Bakugo when he gave Deku the nickname, its also a Japanese pun based on how his name is spelled in Kanji.
