So I finally did my commission sheet, I don't know why it took me so much to make one haha but I'm not too good doing this type of things. You guys think its good like that?

I think this looks great, super straightforward with the examples for each and is simple to understand, one additional option if you'd like to add at the bottom is a wont do category if there is any specifics that you would like to avoid. And again that's totally optional, with or without it I think this a really solid commission sheet!
