So... everyone that encontered Elena before the surgeries (AKA "the accident") must be aware of how radically different (and shorter) she appears after, right? So they must all be in on it, Seferi included. He had an up close and personal discussion with Elena when she was still Andrew in disguise (but aged with chemical injections to the face). And Seferi apparently knows about Andrew, so I think he knows the truth about Elena. I guess the picture is a test?
