I always interpreted Lunge saying "Johan doesn't exist" as him firmly rejecting Tenma's account of him. Unless I'm forgetting something, Lunge never forgets about the child named Johan in the hospital, it's just that he believes the "Johan" Tenma keeps referring to is a figment of his imagination. A convenient cover story predicated on a connection to a former patient.
