This form has never been the 100% state. It has only ever appeared when Mob lost consciousness, but there was no real explanation at first. The first time it appeared it was labeled "???%". He has control of his 100% state, though his actions somewhat depend on the emotion he feels most strongly at the time.

Something I didnt mention about this episode but that ending inner monologue sequence in Mobs mind is chilling I love the imagery of pieces of "Mob" coming off and forming "Kageyama Shigeo"
