I recently watched a great video about this particular episode and the ending of the anime. From what you can gather both Tomoyo and Ushio died in the snow “ Because he was feeling dizzy and obviously wasn’t working because they had like no food” Ushio died but having regrets of her father brought her to the other world. Because Tomoyo Did so many good thing if he was able to have his wish granted and be able to live with his wife and daughter. I can send you the video if you want

Even though it wasn't explicitly shown, I would like to think that he made peace with his father in this reality as well since they still went to see the grandmother. Also that bit at the end with Ushio sleeping under the tree - it's the same tree in the title screen. I mentioned it a while back about the title screen changing little by little; they were actually the light orbs and as more of them appeared on the screen, they resemble a person laying down by the tree xD Speaking of that end bit, in the opening song at the very beginning of Clannad (not after story), there is a brief moment where you can see this end scene as well, with Fuuko finding Ushio laying down

As an additional note you remember how Nagisa explained the the end of Clannad ep 22 how she remembered the rest of the story? She said the story ended with the doll and girl going on a journey( the trip Ushio and Tomoya took on ep 18) to a livlier and warmer place( their relationship that saved Tomoya). They go on a long journey and then.. they sing, the dango song. This is exatcly what happens at the end of this episode. I think it is really cool the second time you watch this series a nd see this detail that you can only understand if you have finished the story.

You said something about Dragon Balls in your reaction to episode 20, but you forgot about the light orbs the show’s been showing you the entire time, huh? No sir, Clannad might not carry any Dragon Balls, but these nifty little wish-granting lights work just as well for all your bring-your-dead-wife-back-to-life needs, Mr. Candy! I hope you’re satisfied with the results or your money back guaranteed! 🤣 So, that's the conclusion to the main story. You seem to be very happy with the ending, that’s good. 😊 Yeah, it’s a touch confusing on how Ushio had to “stay” in the Hidden World, but I think what happened is that she needed to stay in there long enough to make the wish that would bring her family back, and then she could go back too.

Another reason why the ending needed the lights to work is because in the original visual novel you played different "routes" where you dated or followed different girls. To unlock the true ending, the one we got this episode, you needed to play all routes and get the lights at the end of each route when the girls got their wish. When you had collected each light you could know change the ending of Nagisas route from she and Ushio dying to teh ending we got now. p.s haven't played the visual novel but this is how I understand it works.

Clannad ended last episode. Everything else is just extra content
