Gluttony's innocence makes him even creepier. He's obedient, loyal, and simple-minded but will also eat you if given the chance 🍴 In the scene with Al he's extremely scared without Lust or Envy and fears punishment from Father, showing he's basically a hungry child

I know I'm late, but I'm pretty sure you're dead on with the medical condition bit. The community consensus seems to be that he has a super fast metabolism and possibly hypoglycemia. That would explain why he has tons of energy, but needs to eat frequently or risk passing out.
