Were Beth and Miriam the first in the District to get as big as they are? 

No, there is a giantess/giant neighborhood for really BIG people. My plan was that Miriam was to move there temporally until she learned how to change her size.

Was Miriam's latest outing where she outgrew a building relatively recent in comparison to Becka/Tasha?

hmmm I have to confess I did not thought about it, but they could know her quite easy

 How well known are they? Would, say, Bonnie or Adrianne know of their superhuman stature? 

They are quite famous, even after alll the years that had passed they still are celebrities, specially if you are "in the know"
