What would be an example of actual malice in a defamation case, specifically if the organization didn’t know they were spreading a lie? I have a hard time picturing a “reckless” disregard for truth that’s somehow worse than “regular” disregard.

Regular disregard would be to just say something and not care either way whether it is true or not. Reckless disregard would be disregarding the truth in the face of facts that prove its truthfulness. Like continuing to publish lies even though you've been told those statements are false.
