To answer some of your questions: Linda Blair was 14 when the movie was released, so younger than that during the filming. It wasn't her first acting job, but might as well have been. She actually did get hurt during the filming of the scene with her flopping back and forth on the bed. As you already saw, the movie was based on a book, and that book was inspired by a supposedly true life story of the exorcism of a boy in Maryland in the '40s. The medicine that Merrin was taking was nitroglycerin, used for heart issues. Back when this movie was made, that was actually a pretty common movie language element used to quickly convey "This man has a bad heart." Those stairs actually do exist in the Georgetown area of Washington, DC, where a lot of the external scenes were set (and shot.)
