New contact form (for old packs and everything else!) (Patreon)

After a lot of hiccups with the Patreon IM system, I've decided to create a new system for people to contact me: one where I can be absolutely sure the messages and requests get across!

This system will also be the main channel through which patrons on the highest tier can request old packs. From now on, all requests for old packs should go through it!

The advantages of this:
1. It's more reliable, less likely to have messages get lost.
2. Time-stamps on the messages let me know when people contacted me, so I can reply in order in which each request was sent.
3. A friendly drop down menu so you can pick old packs.
