Depends on how the story supports them. If there is a fantasy element to the story real is good. If not Fake over the top neko girls are awsome.

I may say both but in reality FW isn't a fantasy setting so roleplay fake neko ears only by my books

It wouldn't look good in your game to have a girl with actual cat ears with the setting. The chances of surviving being struck by lightning is more believable in your story than a girl with actual cat ears.

Well, I like Nekos (real cat ears and tails) but in the setting of FW, the roleplay "fake" ones would be better suited in my opinion.
