Not sure if you've seen it since, but you mentioned that muscle memory hitting square to attack - you can actually change to other more traditional control configurations in the menu, including one that uses square/triangle for light/heavy attacks rather than the shoulder buttons.

Loving the playthrough so far. The move you did by accident at 1:06:25 was your heavy runic attack (L1 + R2). Also, your light axe throw (L2 + R1) travels a bit quicker than your heavy axe throw, which should make ravens and timed puzzles a little less annoying.
