There were 2 trains. The 1st train had Derek (this is when Michael said the humans were escaping). 2nd train was driven by Bad Janet if I'm not mistaken

That's right. The first train had only Derek on it and went to Mindy's Medium Place. Its departure made Shawn think the humans had all escaped there, so Bad Janet called a second train to take Shawn and all the demons back to the Bad Place.
