If you have either already filled out the form and don't see your name OR see your name but the location is either incorrect or outdated, you must either A) let us know in the comments to this thread or B) send us a private message here on Patreon.

VERY IMPORTANT: Please include the name you wrote on the form under NAME AS YOU'D LIKE IT TO APPEAR and your City, State when requesting changes. If your name's Michael Smith and you tell us your info's wrong, we have nine Michaels, eight M. Smiths, and twelve guys that go by Mike S, so please be specific.
