Apparently - and I'm not Japanese, so I'm trusting Wikipedia here - 'futa' is short for 'futanari', which is the actual Japanese word for hermaphrodite or even androgyny - so the original term generally refers to having both sets of genitals. Outside of Japan, it's used to describe a genre of porn involving hermaphrodites, so strictly speaking, a 'futa' is a hermaphrodite, not a sissy, she-male or MtF, pre-op transsexual, BUT language evolves, and we call things what we do. 'Gay' used to mean 'happy'. We all know what we're getting when Kannel does the 'futa' version, and personally, I'm always secretly waiting for it, and I'm always happy when it appears.
