Through the crow he's giving her hallucinations, just like the dream he gave to Elena as the crow and the one to Stefan (although he was there in person with Stefan). Great reaction. and about him being a vampire. does she only remember when he is with her and forget that fact when he is not around? because she asked him why does he not sparkle like Edward in twilight so clearly she knows who he is.
