Ed, Al and Teacher thought that they did bring their mom and baby back with their souls intact. So they thought that their loved ones died and then suffered again after coming back to life as a monstrosity.

This might clean up some confusion toward the end. Ed and Al needed to create a new body for their mother to revive her. There would be no sense in reviving her in her body that she died from being sick. So when the transmutation failed, everyone was convinced that they had caused the soul of the one they were trying to bring back to suffer a second death by failing to figure out how to recreate an Entire person, ie 1 body and 1 soul. Now that they know it is impossible to recreate a body and soul, Ed realized that him being able to bond Al's soul to the armor means he didnt recreate Al's soul, he pulled it from "the Truth". and if he did that then its likely that his body still exists there seperately, like how Barry the chopper existed.
