Generally speaking, Cohles level of introspection would disable a person from his normal functions. Existence requires a certain amount of being numb to the truth in order to operate. Being this sober about life would be debilitating. MM prob needed therapy after playing this character.

Dallas Buyers Club is about HIV/AIDS not cancer. Based on a true story early in the epidemic, when even many/most doctors were treating them horribly and there was no approved treatment. A lot of stigma, fear of it being easily contagious by touch etc. Its a very good movie, though from what I remember also hard to watch at times.
