It seems like the high table is more a parent organization uniting various lower tier crime organizations, such as the Camorra (mentioned in ch2) or wicks upbringing the Ruska Roma. Those organizations my have their own standards but they are expected to always be subordinate to the high table. Wicks problem was not completing his marker. It was going after the brother and killing him in the continental violating their various rules. They definitely stretched the limits of logic on this one that possibly over complicated it some.

As others have said you have to suspend your disbelief and enjoy the world class action scenes and embrace the awesomeness of the idea of a society of assassins while we go about our hum drum lives. I consider this a fantasy and logic cannot be applied to fantasy. The only logic is the logic of the the world that the story is based in.
