In MHA's defense their prisons have only had one break out compared to American comics and it took the two of the most powerful villains working together to create an EMP to shut down all the prisons defenses to do it. Before that they were able to successfully keep super villains safely imprisoned. Even now all the prisoners they recaptured are being safely held. It is not that the defenses are bad, but rather they weren't prepared for enhanced Shigaraki.

I know a lot of talk is made about All For One’s plethora of quirks, but this episode clearly showed that his most powerful quirk is “Troll”. It gives him the preternatural ability to troll quite literally anyone and emotionally rattle them to get them off focus. There is something truly entertaining about a guy who read a comic book then got inspired to become a demon lord who just wants to make the world his playground while trolling heroes till his heart’s content. All For One was made for Twitter. Also his quirks were further evidence of Horikoshi’s horror talents. Instead of the usual powers we see in superhero stories we get some grotesque body horror quirks. It really matches well with All For One’s shit talking vibe.
