To explain Gae Bolgs noble phantasm, the reverse of cause and effect: the effect is piercing the enemies heart, so it will alter reality (the cause) to make it happen. If I remember right, the reason Saber could avoid the altered reality is that her instinct is on a level were it can go against fate/reality.
