4th Gear is for sure my favorite Gear... it looks so sick and the reveal is so iconic. As for why Luffy didn't use it in other arcs, you will have seen by now that it's because it's a very short duration form that is his last resort. If he can't finish the enemy in 4th Gear then in most situations he loses and all his friends would die, it's not to be used lightly. It wasn't needed in his other fights like you guessed. The biggest risk with all or nothing forms that run on a short timer is your enemy realizing they can't beat you and escaping or hiding until you lose the form... then coming back to kill you and your friends when you are helpless from the after effects. With Hordy Jones he was so weak on land that it would have been complete overkill and a needless risk, and in the water there is just no point since he was restricted to his little bubble and Hordy could escape him even if he got a hit in. Caesar's abilities make him extremely dangerous to use 4th Gear against for two reasons. One, if Luffy had used 4th Gear in their first fight Caesar would have potentially still suffocated him and defeated him by catching him off guard with that ability. Second, Luffy would have to finish Caesar in one go because Casar being made of gas could just escape through cracks in any location to where Luffy couldn't follow him once he sees how strong 4th Gear was. I think Casar could tank 1 4th gear attack probably, two maybe not. That's why they say to save your trump card after all the cards are on the table, because your opponent may have a way to counter your ability if you are ignorant of their powers (namely devil fruits). These are "what if" scenarios but in a life or death fight where everyone's lives are on the line that's not the time to take any risks. In practice it's possible 4th Gear would have one shot Hordy and Caesar before they could do anything if you scale it to be that powerful (I don't, I think it would take 2-3 hits to down them), but why take the risk when he was already way stronger than his opponents without it? Doflamingo was just the first opponent he stood no chance against without it. Even with 4th Gear he would have still been killed if not for the intervention of others.
