Really nice reaction. Good questions. Don't know the answers. Roy healed both believers and non believers. It didn't matter to him. But seems that the wife had some say. A lot of people who watch this episode comment on the chemistry between Jensen and Julie. They are both really good actors and do those subtle nuanced performances so well. When there's another really good actor in a scene with him, it is just a beauty to watch. Julie is quite supreme and I'm sure brought something special to the show that Jensen fed off of and added to his performance. You're right, Dean did care about Leila. He seemed to be willing to let the Reaper take him, so Leila would be healed. He didn't even try to run from the Reaper. I could tell that this whole thing really bothered Dean. That someone else died so he could live. And then that Leila didn't get healed because the reverend chose Dean over her. He didn't want anyone else to die though, so if the reaper took Dean but Leila got healed, I think he would have seen that as a balance. It did seem to be a moral story in this episode too. Like with the scarecrow episode, the guilty people manipulating the god got punished in the end. Same with the wife here. It was really nice to see how hard Sam was working to save Dean. The episode was a chance to see some more on the differences between the brothers. I think Dean likes to feel like there are clear black and white rules to things. I think it just makes life easier that way. It's complicated enough and the job is hard enough. Why make it more difficult. But it is odd that he is so determined to not believe some things, since he knows that there are supernatural things out there. So why not believe in religion, which really is really only different in the trappings it's wrapped in.
