I’m pretty sure Jon Bernthal actually hurt his hand during the filming of this season so they just worked it into the script. He wears that cast for most of the season

There was a lot of disappointment about Billy’s not so mangled face considering how it looks in the comics. The writers explained they were going for a less literal interpretation of the “Jigsaw” moniker and focusing on the psychological aspect. We see Billy covering his face a lot with his hands or the mask even though the scars aren’t that bad so it’s more about his internal trauma about what happened to him and less so how it looks to everyone else.
