I'll admit, I don't know much about Danganronpa, but is Ultimate Despair just when someone goes feral in that series?

well, i have to premise this by saying it's a big Spoiler and you should not read on if you intend to play/watch the series. Basically in this instance it's the result of a form of brainwashing that leaves the victim without any hope and drives them to obey their basest instincts or commit any kind of atrocity to spread their despair to others, or even to cause misery for its own sake
