Great episode, shame that they cut so much comedy from manga, but it does help to give more serious tone after defeat, and shame that they cut mayuri explanation why exactly he can't repair ichigo bankai since we saw few of them destroyed and repaired already.

They said it in the episode that they were changed by soul king. Their bones act like keys for example. So I am gonna assume that when a gotei 13 captain gets chosen for promotion to royal task force squad zero, they get a buff from soul king after getting the position which makes them much stronger than a usual captain.

I believe Orihime has the best healing. But she was in hueco mundo right now and Unohana was not gonna be able to heal Byakuya and squad zero was confident that they could. So I guess it is easy to say, why Orihime did not take the job. Squad zero took over before she could.

Orihime was never asked for healing Yama's arm. Yama never asked for it cuz of pride. Him not wanting to take help from humans and wanting to deal with their problems on their own.
