Khan, when shimura says "if that man finds out Kotaro exists, he'll definitely use him", she meant if all for one finds out her son exists he'll use him. Kotaro is shigarakis father. So the grandmother, previous one for all user, changed her family records so no one would know she had a child, and so all for one wouldn't be able to harm him or use him as leverage against her.
