If you do Ugly girl one, You have to show her after each wish so we can see the effect. Also have her get more desperate.. By the last , have her "I wish I looked like a bimbo" and the fountain takes the wish literally and make her look and think like a bimbo.

I love the surgery one, if the wish one works I hope she will gradually be getting lip implants and waist reduction to complement her expandable breast implants that are filled successively, as she ignores the Victoria secret girls who keeps measuring her into bigger and bigger bra sizes as she bubbles out of her current size with tighter, shinier, newly stretched titskin.
