It's quite simple, really. Those people aren't falling down because of an illness. They're getting lead poisoning, and as we all know, lead poisoning attacks the central nervous system. That wheel rim rolling down the hill reminded me of that scene in "It's A Mad, Mad, Mad, Mad World" when the old man kicked the bucket, and in doing so he kicked a bucket that slowly rolled and bounced down the mountainside. This game is so fun to watch you play! Watching a religious cult get splattered is as fun as watching nazi characters get splattered in the other games you've played.
