This proves the point regarding the need for check lists and regular refresher training of all staff. In the fire service we were trained to treat every incident as unique, but had a series of check lists for almost every type of incident. We understood the need to adhere to our training in dealing with incidents but to be alive to every development and take nothing for granted. These videos are excellent examples of the need in sharing information to prevent problems being repeated.

This could’ve turned out to be a very very big deal. Not that it wasn’t already however should this aircraft not have had enough airspeed to fly, many if not all would’ve been killed over a decision made by these pilots.
