On the 25th of May 1979 American Airlines flight 191, a McDonnell Douglas DC10, with destination Los Angeles, took off from Chicago O'Hare International airport. During the rotation, the nr1 engine detached from the wing causing extensive damage to several aircraft systems, eventually leading to loss of control of the aircraft.
