Might be a bit redundant as it goes a bit into another idea listed but having a NBM woman (4 arms/4 breast) wearing a headless horseman outfit/cloak where she is using her hands to hold a jack-o'-lantern where her head should be. She would be holding her head in her lower set of arms. If you would want to simplify the idea of just having a "normal" NBM womanlike the above idea of Brittany being a headless horseman you could have her holding a jack-o'-lantern where her head should be and her head is on the ground that could merge the two ideas together.

Actually the more I thought about it if you are going to be a headless horseman might as well be the horse itself :P. The idea will be better if the horseman idea was a humantaur! At that point instead of having 4 arms so she could hold her head you could have her head on where the saddle should be on her lower body back. Or keep the 4 arms but I am not trying to complicate the idea which isn't going to well :)
