You ask "How does a country operate like this?" The unfortunate truth is that all countries operate like this. Politics takes precedence over science in all countries. Most people don't believe in what their own senses don't tell them, with the single exception of religion.

This accident could not happen. This was not possible. IF the design was flawless and IF the construction was flawless and IF the operation of it was flawless. Honest mistakes and cutting corners and outright corruption. Between these three, the former three could not be flawless. This is true in every country. If you are going to operate technology which could result in mass destruction, you need perfection. And when in human history have we been perfect?

There were slight inconsistencies of reality in the series, but only one seems significant to me. I'm talking about the moment when miners patted that jacket of minister. This is completely contrary to the worldview of Soviet citizens. This really was not, even some miners mentioned this in an interview.
