Every reaction I’ve seen has the exact same question on this episode; “why did Mami die?”. If you look at her magical girl form, her soul gem is a hairpin. When she got decapitated the witch ate her head, instantly crushing her soul gem. She took the ONLY lethal injury to her, without ever knowing that was the weakness. Literally anything else wouldn’t have killed her there. Which makes it all the more fucked that Kyuubey doesn’t tell any of the magical girls, because so many of them just present their soul gems out in the open. They don’t know to hide or protect it, so an unlucky glancing blow to the gem can take them out.
